Overview
The Clinical Care Coordinator plays a vital role in improving health outcomes for Aboriginal and Torres Strait Islander people through preventative health strategies, chronic disease management, and culturally responsive care. This role supports clinical service delivery, health promotion, and community engagement, while coordinating mobile outreach and follow-up care.
The role is based at both Halls Gap and Ararat, with regular travel between both sites as required by service needs.
About you
You are a Registered or Enrolled Nurse with current AHPRA registration. You will have demonstrated experience in working with Aboriginal and Torres Strait Islander communities; an ability to communicate and build connections with Aboriginal and Torres Strait Islander communities; and demonstrated experience in primary heath care, preventative health, or chronic disease management.
