Overview
The Cultural Care Coordinator provides intensive non-clinical support through engagement, cultural sensitivity, assessment, advocacy, advice and case management. The role is pivotal in improving health and wellbeing outcomes for Aboriginal and Torres Strait Islander people and families through preventative health strategies, care coordination and case management, in a culturally responsive way.
The role is based at both Halls Gap and Ararat, with regular travel between both sites as required by service needs.
About you
You will have knowledge and understanding of health and wellbeing issues impacting Aboriginal and Torres Strait Islander people; demonstrated experience in working with Aboriginal and Torres Strait Islander communities; and demonstrated experience in primary health care, wellbeing services or preventative health.
Qualification in Case Management, Aboriginal Health, or Social Work is desirable.
